October 13 - Deadline to sign up
*November 18, 5:00pm - Administration of Equipment at Elementary School
*November 20, 5:00pm - Optional Make-Up Date for Equipment at Elementary School
*March 9, 5:00pm - Return Equipment at Elementary School
*March 11, 5:00pm - Optional Make-Up Return Equipment Elementary School
*Parent must attend in order to sign the equipment agreement

After this date, it is not guaranteed that an athlete can play for any practices or in any games for our parish.
The following forms MUST be completed and submitted to Final Forms by October 13:
1) Concussion Awareness Acknowledgment
2) Emergency Medical Authorization Form
4) Sudden Cardiac Arrest / Lindsay's Law Information and Acknowledgement Form
